The Care Connection Aging & Disability Resource Center [ADRC] is your trusted access point for information, resources, and services that support older adults, individuals of all ages with disabilities, and their caregivers. Our goal is to make it easier for you to find the help you need-quickly, reliable, and at no cost.

Care Connection provides a convenient and user-friendly way to navigate community programs, long-term services, and supports. Whether you are seeking assistance for yourself, a family member, or someone you care for, our team is here to assist you.

We are funded by the Texas Health and Human Services Commission [HHSC] and operate in partnership with the Houston Health Department’s Harris County Area Agency on Aging. Through these partnerships, we work collaboratively with community organizations and professionals to serve residents across the Harris County region.

Our mission is to assist any person with a disability, veterans, older adults, and caregivers in accessing the services and supports they need.

We are committed to making service navigation easier, less fragmented, and more connected through strong partnerships and coordinated resources.
