• HOUSING NAVIGATION: Services designed to help individuals and families identify appropriate, affordable, and accessible housing options.
• NAVIGATION: Assists customers by screening for needs and directing them to the appropriate services and support.
• OPTIONS COUNSELING: Provides support to ensure and monitor service linkages to address long-term and complex customer needs. This may include referral to organizations that provide case management.
• RESPITE: Gives caregivers the time to rest, recharge and refuel, so they can continue to provide care to their loved ones.
• TRANSPORTATION: Provides non-emergency medical rides for people with disabilities and older adults
