About Housing Navigation Services

Care Connection Aging & Disability Resource Center [ADRC] provides housing information and navigation support to older adults and people with disabilities, empowering them to make informed choices about where and how they live. We recognize the significant need for additional affordable, accessible, and integrated housing options throughout Harris County and actively work to build relationships with housing providers to encourage the expansion of housing opportunities within the community.

Our Housing Navigation Services are designed to connect individuals and families to resources that support stable, safe, and sustainable housing while promoting independence and community living.
 

Transitioning From Facilities to the Community
Non-Medicaid consumers who are exiting a licensed nursing facility or licensed assisted living facility in Harris County and wish to transition back into the community may be referred to the Houston Center for Independent Living [HCIL]. Through this referral, individuals may be eligible for housing navigation support and the possibility of receiving a housing voucher through the Houston Housing Authority, based on availability and eligibility requirements.

In addition to providing information on affordable, accessible, and integrated housing options in Harris County, Care Connection ADRC also offers guidance and referrals to:

Harris County Housing Options

Local Public Housing Authorities: 
Administer subsidized housing programs for eligible low-income individuals and families. Options include public housing units and the Housing Choice Voucher Program [section 8], which helps participants rent housing in the private market. Eligibility is income based, and households generally pay 30% of their income toward rent. Waitlists are common and may open periodically.

Additional Affordable Options: 
Some property owners and developers offer rental units that are not administered through a Housing Authority. These properties set their own eligibility criteria and application processes. Applicants should contact properties directly for income limits, availability, and waitlist information.


Low-Income Tax Credit Properties: 
Low-Income Tax Credit Properties provide housing to low- and moderate-income renters at below market rates. These properties are privately owned and managed, and applicants must meet specific income guidelines. Most low-income tax credit properties maintain waitlists.


Essential Housing Properties: 
Essential housing is designed for moderate-income households, including essential workers. Rent is typically more affordable than market rate housing but higher than deeply subsidized programs. Eligibility requirements vary by property, and waitlists are common.


USDA Rural Programs: 
A federal program funded by the U.S. Department of Agriculture Office of Rural Development provides housing options to serve low-income individuals and families in eligible rural areas. Income eligibility applies and applicants should contact properties directly for details.


Supported Housing: 
This is affordable housing that allows people with disabilities to live independently with help.


55+, 60+, Active Living or Senior Communities: 
These communities are age restricted housing options for older adults who do not require daily assistance. Costs may vary and may include market rate or income restricted options.
