The Houston Department of Health and Human Services (HDHHS) will sponsor five eligibility certification sessions in January for its family planning, maternity and well child services.

The sessions will be from 8 a.m. to 3 p.m. on:

• January 10, Lyons Health Center, 5602 Lyons Avenue
• January 17, West End Health Center, 190 Heights Blvd.
• January 24, Northside Health Center, 8523 Arkansas
  Third Ward Multi-Service Center, 3611 Ennis
• January 31, Sunnyside Health Center, 9314 Cullen

Eligibility applicants will need to present photo identification and documents providing proof of income, Texas residency, insurance, marital status and dependents or family structure.

Also, HDHHS on January 20 will relocate all well child services to two health centers and begin providing family planning and maternity services on selected weekdays. Services are available by appointment.

Well child services will be available at La Nueva Casa de Amigos, 1809 N. Main Street, 713-547-8000, and Sunnyside Health Center, 9314 Cullen Street, 713-732-5000.

Maternity and family planning services will be available either three or four times a week at certain health centers and every weekday at Sunnyside and the Magnolia Health Center, 7037 Capitol, 713-928-9800.
