Qualified Applicants for Vital Records
Documentation requirements are subject to change and must meet the approval of the Registrar. For security and compliance with State of Texas policies, additional documentation beyond the listed requirements may be needed.
Proof of Eligibility
- Each applicant must present a valid primary identification along with their application. If the applicant is unable to present a valid primary identification, two secondary forms of identification are required. If the applicant is unable to present two secondary forms of identification, one secondary form of identification and two supporting documents are required.
Self, Spouse, Son/Daughter, or Brother/Sister
- Valid primary identification
Parent
- Valid primary identification
- Parent's name must be listed on certificate
Grandparent
- Valid primary identification
- Related parent’s name must be listed on certificate
Legal Guardian
- Valid primary identification
- Certified court order showing legal custody
Attorney
- Valid primary identification
- Letter on letterhead establishing current client relationship
- Bar card
